Clinically Isolated Syndrome (CIS) is the first episode of neurological symptoms due to inflammation or demyelination in the central nervous system (CNS). It is often considered a potential precursor to multiple sclerosis (MS).
However, certain infections, notably Epstein-Barr virus (EBV) and SARS-CoV-2 (COVID-19), can trigger or mimic CIS, and their neurological presentations may overlap with MS or other demyelinating disorders.
With shared autoimmune mechanisms such as molecular mimicry and inflammation, the neurological effects of these post-infectious patterns are immense. They lead to immune dysregulation and share structural similarities with myelin proteins, prompting an autoimmune response that damages nervous system myelin. It is, therefore, crucial to have differential diagnosis with MRI.
